What Is Couples Therapy? A Comprehensive Guide to the Process, Methods and Expectations
What is couples therapy, who is it for, how does it work and what should you expect? Expert Psychologist Yasemin KAYA explains the process, evidence-based methods and realistic expectations step by step.

Relationships are among the most meaningful bonds we build — and also the ones that require the most care. Where two different people, two different histories and two different "family cultures" try to build a shared life, occasional gridlock, recurring arguments and stubborn distances are inevitable. Couples therapy is an evidence-based, structured support process that steps in precisely at this point. In this guide, Expert Psychologist Yasemin KAYA explains what couples therapy is, who it is for, how it works and what you should realistically expect.
What Is Couples Therapy?
Couples therapy is an evidence-based form of psychotherapy in which two partners in a romantic relationship work together, guided by a trained mental health professional, to understand and repair communication problems, recurring conflicts, trust ruptures and emotional disconnection. The goal is not to determine "who is right," but to make the cycle within the relationship visible, to render each partner's emotional needs understandable, and to realign the couple toward a shared "we."
In individual therapy the focus is one person's inner world; in couples therapy the real "client" is the relationship itself. The therapist represents neither partner, but the bond between them.
What Couples Therapy Is Not: Common Myths
- It is not a "referee's bench." The therapist does not decide who is right or take sides.
- It is not only a "last resort." Coming earlier yields more lasting results with far less effort.
- It does not aim to "save" the relationship at any cost. Making a healthy decision — including a respectful separation — is a valid outcome.
- It is not a "blame session." The aim is to change the repeating pattern, not to dig up the past.
- Going to therapy does not mean the relationship has "failed." It is one of the most mature signs of investment.
Who Seeks Couples Therapy?
- Premarital couples wanting to discuss money, children, boundaries and intimacy before they become crises.
- Couples in their early years struggling with the post-"honeymoon" reality of partnership.
- Long-term and married couples caught in the same arguments, losing closeness.
- Couples with children who have lost the "partner" identity to the "parent" identity.
- Couples in a trust crisis after infidelity, distance or financial secrets.
- Long-distance couples joining together via online therapy.
- Couples at a crossroads seeking clarity on whether to stay or separate.
Which Problems Does Couples Therapy Help With?
- Communication breakdowns and recurring conflicts
- Trust ruptures (physical, emotional or digital infidelity, lies, financial secrets)
- Decline in emotional and sexual intimacy
- Life transitions (marriage, first child, relocation, job loss)
- In-law tensions and boundary issues
- Parenting disagreements and money conflicts
- Separation and divorce (respectful separation and healthy co-parenting)

Main Methods Used in Couples Therapy
The best-evidenced approaches include Emotionally Focused Therapy (EFT), the Gottman Method, Integrative Behavioral Couples Therapy (IBCT), Imago Relationship Therapy and Solution-Focused Brief Therapy. Our couples work is built on the Gottman Method and EFT. Learn more in What Is Emotionally Focused Therapy (EFT)? and Couples Therapy with the Gottman Method.
The Couples Therapy Process: Step by Step
A typical process moves through: (1) a free intake call, (2) a joint assessment session, (3) individual sessions with each partner, (4) a feedback and plan session, (5) an active working phase, (6) a mid-process review and (7) consolidation and follow-up. For a minute-by-minute look at the first meeting, see "What Happens in the First Couples Therapy Session?"
How Does a Session Go?
A standard couples session lasts 75-90 minutes — longer than individual therapy because both partners need space. The therapist often has the couple speak to each other while keeping the dialogue safe, slows down moments of escalation and assigns a small practice task at the end. Sessions are usually weekly at first, then biweekly.
How Long Does Couples Therapy Take?
A focused single issue often takes 8-12 sessions; established conflict patterns 12-20 sessions; trust repair after infidelity 20+ sessions. Couples therapy is not a years-long process — the work couples do between sessions matters most.
Online or In-Person?
Done well, online couples therapy can be as effective as in-person and is ideal for long-distance or busy couples. We offer both: in-person in Maltepe and online across Türkiye and abroad.
What If My Partner Doesn't Want to Come?
Couples therapy can begin with one partner. Working on the relationship dynamic is possible even when the other partner isn't in the room, and they often join later as change begins.
FAQ
Is couples therapy the same as marriage therapy? Essentially yes; "couples therapy" is the broader term covering all romantic relationships.
Will the therapist take my partner's side? No — the "client" is the relationship between you.
Is what I say confidential? Yes, under professional ethics and data-protection law.
Will therapy end our relationship? It makes reality visible; some couples choose to stay, others to separate well.
