What Happens in the First Couples Therapy Session? A Step-by-Step Guide

Deciding to go to couples therapy takes courage. But the real anxiety often begins at the door of the first session: "What will they ask us? Will we be judged? What if we argue there too?" These questions are completely normal. This article opens the curtain on the first session — from preparation to aftermath — step by step. When you know what to expect, most of the anxiety dissolves on its own.

Why Is the First Session Different?

The first session is not a "treatment" session but an assessment and getting-to-know-you session. The therapist isn't offering solutions yet; they are mapping your relationship, seeking answers to three questions: Why did this couple come? What cycle runs between them? And what goals do they want to work toward?

How to Prepare

  • Ask yourself "why am I here?" in one concrete sentence.
  • Prepare a list of feelings, not accusations ("I usually feel..." rather than "he did...").
  • Clarify your expectation: repair, clarity, or better communication?
  • Handle practical details (for online, both in a quiet, private space, ideally the same room).
  • You don't have to "say the right things" — the therapist's job is to see the underlying pattern.

The First Session, Step by Step

1. Welcome and setting the frame (first 10 min): the therapist explains the process, confidentiality and neutrality.
2. "What brought you here?" (10-25 min): each partner speaks without being interrupted.
3. The relationship's story (25-45 min): how you met, what drew you together, when things changed — also revealing strengths.
4. Making the cycle visible (45-70 min): a recent argument is examined to see the pattern, not to assign blame.
5. Goals and next step (70-90 min): what you want from the process; usually individual sessions follow.

What the Therapist Asks

"How did you decide to come?" "What would you most want to change?" "When did you last feel truly close?" "How do your arguments start and end?" "What is your relationship's greatest strength?"

What the Therapist Assesses

Your conflict style, repair attempts, emotional reactivity, attachment cues and existing strengths — which determine whether EFT or the Gottman Method will lead.

Individual Sessions and the "Secrets" Policy

After the joint session, the therapist usually meets each partner alone. A key ethical rule: information shared individually is not kept as an "undisclosed secret" that would undermine the couple's trust. This is explained upfront.

Common Feelings in the First Session

Anxiety, defensiveness, relief, unexpected tears and hope — all are normal.

"What If We Argue in Session?"

This common fear is the least necessary. If tension rises, the therapist sees your cycle live and teaches you to slow it down safely. The therapy room is the safest place to argue.

After the First Session

Some couples feel lighter and hopeful; others feel temporarily more unsettled when long-suppressed topics open. The latter is not alarming — like a wound stinging as it's cleaned.

First-Session Checklist

Can you state why you came in one sentence? Will you try "I" language? Are practical details ready? Do you accept the therapist won't take sides? Are you willing to try a few sessions?

FAQ

How long is it? Usually 75-90 minutes.
Are we together? Yes for the first assessment; individual sessions may follow.
Must we continue after? No — it's also your chance to assess fit.
